Julius Fernando/ Physiotherapist

Julius is a licensed Physiotherapist in good standing with the College of
Physiotherapists of Ontario. He is a member of Vestibular Disorders Association and is
trained in the assessment and treatment of dizziness and balance disorders. Julius is
also trained in Lumbar Spine Mechanical Diagnosis and Therapy from the McKenzie
Institute Canada for cases like lower back pain and sciatica. Clinically, Julius empowers
his patients to take an active role in their recovery and guides them towards their goals.
He believes in evidence-based approach to assessment and treatment of certain
conditions and in the power of communication to help patients take over their healing
process.

Suhel Diwan/ Physiotherapist

Suhel has been working in the field since 2016 while also extending his studies to include fitness and health promotion. Suhel prides himself on providing patient centred care and ensures specific recovery program is designed for his patients. Using a combination of manual , exercise based therapy and modalities his approach is to reduce pain, while improving stability, strength and flexibility

Samira Patel/ Physiotherapist

Samira is a well-experienced physiotherapist and health coach, her main goal is to help with pain management, injury prevention, and maintenance of overall health. Samira's therapy approach involves a variety of techniques including joint mobilizations, soft tissue release and
electrotherapy. Her patients benefit from her honest, energetic and motivating approach.
Samira can speak Hindi, English, Gujarati, Urdu Language.

Jenny Mao/Registered Massage Therapist

Jenny has extensive experience working in various settings such as rehab centers, clinics, and spas, which makes her able to accommodate varied clients’ needs. She has experience with deep tissue massage, Swedish massage, sports massage, hot stone massage, aromatherapy, and trigger point release. Jenny cares about the patient’s needs, always utilizing her skills and knowledge to work out an effective treatment plan with the patient.

Eric Meng/ Registered Acupuncturist

Eric is a licensed Acupuncturist (R.Ac) in Ontario. He graduated from Ontario College of
Traditional Chinese Medicine. Eric can perform comprehensive acupuncture treatment including
traditional, electric, scalp, auricular, facial, and cutaneous acupuncture. With the experienced
manipulation, Eric wants to help in pain relief, motion disorders, and various issues that disturb
you in your everyday life. Eric will build long-term care to maintain balance and natural health for
you.

Patient Eligibility for OHIP Physiotherapy

To be eligible for one to one physiotherapy services at Elite Physiotherapy & Health Centre under the Ontario Ministry of Health and Long Term Care (MOHLTC) Public Funded ” Community Physiotherapy Program” a person must, for each ” Episode of Care (EOC) meet the following criteria:

Have a valid OHIP card and a referral from a physician or nurse practitioner
aged 65 years and older OR
aged 19 years and younger OR
any age if recently discharged as an inpatient of a hospital and a physician on the medical staff of the hospital determines that the insured person needs physiotherapy services that is directly connected to the condition, illness, or injury for which the person was admitted to hospital for OR
referred by a physician or nurse practitioner based on an assessment that the person requires physiotherapy services and is eligible under the Ontario Disability Support Program (ODSP) or Ontario Works (OW) programs.

What is OHIP Community Physiotherapy

The MOHLTC’s community physiotherapy program was designed to provide eligible patients better access to physiotherapy services. This program makes our physiotherapy services available to those that may not have been able to access it before.

The program is structured under an Episode of Care (EOC) model that provides for a time limited, goal orientated physiotherapy treatment for a specific health condition. The objective is to rehabilitate physical function and/or promote mobility when that function and or mobility has been impaired as a result of a debilitating event or disease, pain, injury or surgical procedure. An EOC must not be provided concurrently with any other funding source, such as WSIB and automotive or extended health insurance.

Outcome measures will be taken both on assessment and at discharge. As a patient completes an EOC, a discharge report will be sent to the referring physician or nurse practitioner. However, please note that the “Community Physiotherapy Program” is for the treatment of specific acute conditions or acute on chronic conditions and is not meant to be use for maintenance of chronic problems.
